As a new patient, we ask that you please arrive 15 minutes prior to your scheduled appointment time to complete the registration process. It is advised that you complete and print the Patient Registration Form prior to your arrival. This will help expedite your chart preparation and avoid delays. You will then be greeted by one of our ophthalmic technicians, who will escort you to the exam room, take your history information, and perform some preliminary testing on your eyes. Don't forget to bring a list of your current medications and information regarding your general health and any ocular conditions. You will then see your doctor, who will carry out additional testing, explain the results of your examination, and discuss treatment options with you.
It is extremely important to have your insurance information up to date at all times. If your insurance plan has changed, or you are planning to change plans in the near future, please contact our Insurance Coordinator at (206) 812.2991 to make sure we are contracted with your insurance carrier.
Our office is contracted with most insurance plans, however some insurance groups require Referrals and/or Authorizations prior to treatment or office exams. If you have an HMO Plan, your appointment will not be confirmed until authorization has been received from your insurance company.
Ophthalmic Consultants Northwest respects your privacy. We understand that your personal health information is very sensitive. We will not disclose your information to others unless you tel us to do so, or unless the law requires and/or authorizes us to do so.

Dr. Rotkis performs surgery at Swedish Medical Center, Seattle Surgery Center, and TLC Laser Center depending on the procedure indicated. Surgery is usually Out-Patient so you will go home the same day and will be scheduled for a follow up appointment approximately one week after suergery
